Cockroaches Crawling at Snellville Pizza Place

Violations: 1-2, Violation of Code: [.03(2)(a)-(n) ] Imminent health hazard was present today due to cockroaches crawling over exposed food and clean food-contact surfaces. Restaurant has continued to operate under these conditions. Matters today, regarding pests, were the worst observed in the facility thus far. Roach infestation has been noted on multiple inspections. However, there has been no visits to the restaurant by pest control company this year. There is also no contract with pest control company for regular services. Management must develop a plan of action to correct the pest control problem immediately and for long-term compliance. The owner and persons in charge must ensure that if an imminent health hazard exists because of gross unsanitary conditions, or other circumstances that may endanger public health, then operations are to be immediately discontinued and the Health Authority is notified. / Restaurant permit will not be reinstated until the restaurant has been thoroughly cleaned and treated; and there is a fully executed pest control contract in effect. See remarks section and violation category 18.  New Violation. 3-1C, Violation of Code: [.04(1) ] Romaine salad greens in the walk-in cooler are in poor condition: brown discoloration throughout the batch. Discard the salad. Foods must be safe, unadulterated, and in good condition. 2ND CONSECUTIVE VIOLATION OF CODE SECTION  Corrected On-Site.  Repeat Violation. 4-2A, Violation of Code: [.04(4)(c)1(iv) ] Foods stored uncovered while not in use: Bulk dry ingredient underneath the dough prep table, diced ham in the walk-in cooler Food not covered to protect against contamination by pests: All of the foods on the pizza makeline are completely exposed to contamination by cockroaches crawling along the underside of counter ledge protruding directly over the pizza makeline. Foods must be kept covered to protect them from contamination when not in use. Also foods must always be covered to protect against imminent hazards. Must cover containers and close packages etc.  Corrected On-Site.  New Violation. 6-1A, Violation of Code: [.04(6)(f) ] The majority of the TCS foods in the walk-in cooler are not being maintained at 41F or less. See temperature observations. Discarded foods such as pizza sauce, block mozzarella cheese, ricotta cheese, cooked meat toppings, sliced tomatoes, diced ham, etc. Foods held cold for safety must be maintained at 41F or less. See violation 15A.  Corrected On-Site.  New Violation. 6-1C, Violation of Code: [.04(6)(d) ] Sliced tomatoes prepared earlier today and bulk container of pizza sauce prepared yesterday have not cooled to 41F within the proper time frame (tomatoes should have been cooled within four hours of prep; pizza sauce should have been cooled within fours of prep from ambient temperature canned tomato sauce). Discard these foods. TCS foods must be cooled from 70F to 41F within four hours (and for hot foods, must be cooled from 135F to 70F within two hours time).  Corrected On-Site.  New Violation. 6-2, Violation of Code: [.04(6)(h) ] Multiple foods held past 7 day refrigerated shelf life: bulk alfredo sauce, bulk sausage topping. Discard the foods. Ready-to-eat TCS foods must be held not longer than seven days in refrigeration. (Also, commercially packaged TCS foods must not be held for more than 7 days after the package has been opened.)  Corrected On-Site.  New Violation. 8-2B, Violation of Code: [.07(6)(l) ] Residential-use pest sprays are on-site and being used to treat pest infestation. Remove from the facility. Only pesticides approved for use in commercial kitchens can be applied in the restaurant and use licensed pest control operator to apply the chemicals which warrant a license for application.  New Violation. 12A, Violation of Code: [.04(4)(q) ] Foods are stored in direct contact with the floor: bulk sugar in the prep area and several boxes of mozzarella cheese inside the walk-in cooler. The foods had been delivered several days ago. Move the food to shelves. Foods must be stored at least six inches above the floor.  New Violation. 15A, Violation of Code: [.05(6)(q) ] The walk-in cooler is not producing air at 41F or less. The ambient air temperature is at 56F. The salad cooler was no providing air of 41F or less. Shredded mozzarella cheese was being held in this cooler. Repair the coolers. Coolers which hold TCS foods must provide air which is 41F or less. The coolers must be kept in good repair.  New Violation. 15C, Violation of Code: [.05(7)(a)2,3 ] Several equipment surfaces still have food residue: sides of the fryer, sides of pizza ovens, roof of pizza oven, under the lid of the salad prep cooler, side of the residential refrigerator, etc. Clean the equipment outer surfaces. The gaskets of the reach-in coolers and residential refrigerator have accumulation of food residue and debris. Inside the residential refrigerator is unclean with food particles at the bottom. Shelves inside the walk-in cooler are coated with food residue.  Repeat Violation. 17C, Violation of Code: [.07(5)(a),(b) ] Flooring is unclean throughout the restaurant: behind the makeline coolers, grill, ovens, residential refrigerator, shelf for clean wares, underneath shelves in walk-in cooler, underneath the vegetable sink, underneath the dry storage shelf for pizza boxes and canned foods, etc. Thoroughly clean the floors. The physical facility must be cleaned frequently enough to prevent accumulation of food debris and dirt.  Repeat Violation. 18, Violation of Code: [.07(5)(k) ] Cockroaches are crawling all along the wall behind the pizza ovens, grill, and fryer; crawling along the counter ledge above the makeline; crawling along the floor and baseboard behind the hot holding foods in the steam well; crawling along the walls and inside the back of the residential refrigerator; crawling along the baseboard behind the shelf where clean dishes are stored; crawling along the wall underneath the vegetable sink; crawling out of the broken wall tile near the manager's office; crawling out of the wall behind the pizza makeline; crawling at the flooring underneath the pizza makeline; feeding on all of the food under the makeline; crawling in the dining room flooring; crawling at the wall and floor behind the pizza dough prep table; crawling at the wall underneath the compartment sink near the corner where bulk flour bags are stored; crawling along the wall underneath the handwashing sink at the side of a pizza oven; crawling at wall near walk-in cooler entry, etc. Complete thorough cleaning of the entire facility to include pulling out all equipment, cleaning floors, walls, and ceilings. Clean every surface and crevice possible of food debris and residue. Then have a licensed pest control company treat the facility. The presence of pests must be effectively controlled in the establishment. The infestation has progressively increased. Improper treatment of a pest infestation can worsen circumstances. Must show proof of a fully executed contract for pest control services from a licensed company to inspector. Also seal all openings in the back door (at the lock and base of the door). THIRD CONSECUTIVE VIOLATION OF CODE SECTION  Repeat Violation. 18, Violation of Code: [.07(2)(k) ] Bait stations for roaches and fly control devices are positioned in locations directly over food prep stations (vegetable sink and makeline, etc.) Remove them from these locations. Insect control devices must be located in a manner which prevents contamination of food and food-contact surfaces.  New Violation. PERMIT IS SUSPENDED due to pest infestation and inadequate refrigeration. As per Chapter 290-5-14-.10-1(b)(3), the facility must immediately cease operation until further notice.
